NOTE: Additions or changes may have been made to these documents since publication on the world wide web. Please consult SSD for current policies.

The student must provide Services to Students with Disabilities (SSD) with disability-related documents from the appropriate licensed professional to certify a student as having a disability and to determine reasonable accommodations. The responsibility and cost of obtaining all documentation is borne by the student. If the initial documentation is incomplete or inadequate to determine the extent of the disability and reasonable accommodations, the office has the discretion to require additional documentation. If the University desires a second professional opinion, the University bears the cost.

Staff from SSD certify that a student has a disability and register students with disabilities for services provided through SSD; students who are found to be non-disabled are referred to other campus and community resources for assistance. Pending receipt of documentation, SSD reserves the right to deny services or accommodations.
